Tell us about your experience with the NDIS

JFA Purple Orange is excited to invite NDIS participants (or someone responding on behalf of an NDIS participant) to take part in a survey.
 
This survey has been carefully designed to gather the stories and experiences of NDIS participants. The findings from this survey will form the basis of a major body of work our agency is undertaking in relation to identifying experiences of NDIS participants as they move along the participant pathway.
 
All data collected will be treated confidentially. You can choose to remain anonymous if you wish, however we provide an option for your contact details at the end if you would like to be contacted by us for further consultation opportunities related to this piece of work.
